Minecraft SMP for dummies?

Ok. Me and a friend have minecraft. I want to host an online server for me and him only (we're not on the same local network). How does he connect to me (I've got the server)? How do the worlds save and where? Can I use the give command, then save a map full of stuff offline? Thanks in advance.
